Two letters of recommendation
Work sample. Provide a sample of your best academic writing (2,500 words). Do not submit work by multiple authors.
If in the arts, a creative work sample may be provided in lieu of a writing sample (no more than five pages or five minutes of audio/visual).
You will receive your award when you and the registrar have fully completed the Certificate of Registration, and it has been received by the national office no earlier than July 1 of the year in which you are applying.
Award recipients are required to submit a W-9 to the Society. Based on this award type and eligibility criteria, the Society will not issue a 1099-MISC to the recipient. However, award proceeds may be taxable to the recipient depending on various factors. Recipients should consult with their tax advisor regarding the taxability of award payments.
* Foreign persons, including nonresident aliens, must
provide Form W-8 BEN. Depending on the award type and the information provided,
Phi Kappa Phi may be required to withhold 30% of the award for tax purposes per
Internal Revenue Service regulations.
